-Dec29-Edmonton. Worker placed truck chassis on jack stands, removing muffler system, chassis shifted, fell on worker, worker deceased

-Dec14-Raymond. Worker removing ice block from auger on ice resurfacer, hand caught in moving auger, broken bones/partial amputation

-Dec15-Calgary. Worker on 2nd rung of ladder wiping down machine, slipped, fell to floor, fractured pelvis.

-Dec12-Calgary. Worker had muffin in hand, coffee in other, boot laces exposed, eyelet hook on boot caught hook on other boot, fell, struck head.

-Dec12-Edmonton. Worker walking in public areas of building when slipped and fell, injured hip, time required in hospital.

-Dec12- Edmonton. Worker refuelling piling rig, slipped from platform, fell 1m, injured wrist requiring surgery.

-Dec13-Hinton. Worker filling 2, 80lb propane tanks at site, explosion, flash fire, minor injuries, treated at scene.

-Dec9-Medicine Hat. Worker on step ladder, lifting item off warehouse shelf, wet boots, slipped, fell with box on top, broken collar bone.

According to the OHS Act, injuries and incidents have to be reported to the Government of Alberta if they:

(a) result in a death;

(b) cause a worker to be admitted to hospital for more than two days;

(c) involve an unplanned or uncontrolled explosion, fire or flood that causes or has the potential to cause a serious injury;

(d) involve the collapse or upset of a crane, derrick or hoist; or

(e) involve the collapse or failure of any component of a building or structure necessary for the structural integrity of the building or structure.
